Returning Medical Documentation

All Patient-Centered Community Care (PCCC) and Veterans Choice Program authorizations issued by Health Net Federal Services, LLC (HNFS) ended Sept. 30, 2018, with the transition of patients to their U.S. Department of Veterans Affairs (VA) Medical Center (VAMC).

Health Net Federal Services is no longer accepting medical documents and the current fax number has been deactivated. Should you need to deliver overdue medical documents for HNFS-authorized services, you must deliver these directly to the authorizing VAMC indicated in the authorization paperwork provided to you.  

Do not fax or mail any medical documentation for veterans in your care to HNFS, unless you are specifically requested to do so for claims reconsideration. HNFS does not have the ability to route medical documentation to the appropriate VAMC.

VA may request medical documentation that cannot be scanned or faxed, for example a CD of images. In these cases, the non-scannable medical documentation must be mailed to the VA requesting the information.

  • Veterans Choice Program: The VAMC address information is located on the 10-0386 for Choice First and waitlist-eligible veterans, and can be found at www.va.gov for mileage-eligible veterans.
  • Patient-Centered Community Care: The VAMC address information is located on the 10-7079 for outpatient services and the 10-7078 for inpatient services.

Important Note: Do not submit claims with medical documentation, as we cannot accept faxed claims for processing.
